Clinical Care Manager

We are looking for a dedicated Clinical Care Manager to lead and deliver person-centred care, ensuring that each service user’s preferences, values, and needs are at the heart of everything we do, while adhering to home support best practices.

Key Responsibilities:

• Overseeing the development of Person-Centred Care Plans: Managing and collaborating a team of Supervisors to ensure that service users and their support networks lead the development of tailored person-centred care plans that reflect each individual’s unique preferences, goals, and needs.

• Promoting Service User Autonomy: Foster an environment that encourages service users to make informed decisions about their care and support them in maintaining independence.

• Staff Supervision & Training: Mentor and train staff in the principles of person-centred care, ensuring that every team member is equipped to treat service users as active partners in their care journey.

• Ensuring Quality & Safety: Monitor care outcomes, ensure high standards of care delivery, and lead quality improvement initiatives that keep the service user’s experience at the forefront.

• Collaboration: Work closely with a multidisciplinary team to coordinate and deliver seamless care that meets the holistic needs of each service user.

• Advocacy: Act as a strong advocate for both staff and service users, ensuring that their voices are heard, and their rights are respected in every aspect of the provision of home support services.

Qualifications & Experience (Candidates must):

• Licensure: Registered on the general division of the Nursing and Midwifery Board of Ireland live register.

• Experience: At least 5 years post registration experience with a minimum of 2 years in community-based role.

• Qualification: Management and home support desirable. Must be willing to undergo further training as required.

• Other: Full current driving licence and access to own car.

Working Conditions

• Monday to Friday 09:00-17:00.

The Clinical Care Manager will be required to work as part of a team. Therefore, a flexible working attitude is required.

On-call as and when required is part of this role – responding to business demands that occur within the person-centred care and support team.
